I wouldn't leave it blank as they may think you have missed a question and send an RFE. N/A is the wrong answer as this would suggest she was conceived without a father. So the best way to answer this is "unknown". I am assuming she doesn't know her father?

If she does not know who her father is, then put Unknown in all of the boxes.

If she does know who he is, then she must fill in his details as best as she can--at the very least his name (not being on the birth certificate doesn't automatically mean the information on the other parent is not required).  Her mother can probably best assist her with the father's information, if known.

She knows her father just by name..so are we just going to put her father’s name? Or is it safer to just put unknown since her father is not in the birth certificate..

If she knows her father's name....would be untruthful/misrepresentation to list "unknown" as the name.....since she knows it :)

For the info she does NOT know, she can put "Unknown" in those boxes.  It's always better to be truthful and fill out the answers as much as you can.  Again, perhaps her mother can assist her with the other information needed about the birth father.

Definitely give the name if the name is known. I assumed (wrongly) that she had no knowledge of her father's identity.
