DS 3025

I read the sticky’d thread but am still a little confused on this. My now wife had her medical done in September 2017 I believe (well within a year) - but in the Philippines, that envelope/CD was kind of like her Visa packet... Never hers to open. Thus, I do not believe she would have any WAY to have her DS 3025/documents on hand to send in with our AOS... 

 

   I have not worked on this form yet, but ummm.. am I supposed to just more or less mark it as “complete” (any questions asked) and hope the USCIS didn’t lose it etc? I can’t think of any point where a medical document would have been given back to her.. She had the required vaccinations, to our knowledge (standard St. Luke’s medical for K1 visas)... but without ever seeing it with our own eyes, how do we know all of what she did/didn’t get??

You get your own copy of the DS-3025 from the doctor at the medical (if they don't automatically give it to you, you ask for it), that's the one you take yet another copy of and send with the AOS package. The original DS-3025 is in the visa package, you'll never see that one.

In addition to Scandi's comment, no you don't mark anything complete. The panel physician would have marked the DS3025 as complete  if he deemed it as complete.

Some forget to give beneficiaries the copy of the DS3025 and then you have to ask for it.

If the Dr. forgets... I mean, do we just not submit that form and “hope” they have it on file? Should I not submit that form and write a note instead? Unsure on what to do since she had it not too long ago -

The doctor doesn't really care if she gets a copy or not, it's not his future. It's up to your fiancé to ask for a copy if she's not given one automatically. Have her contact the doctor and see if she can get a copy now, several months later. She might be lucky.

You may well have to get the I-693 completed at this point.  You could try contacting the medical exam facility and see if they kept a copy or can at least forward the vaccination record.

There are things the beneficiary must make sure they obtain and bring with them to the USA ie, DS3025, certified copy of birth certificate, etc are useful, needed documents

You have not received a RFE yet so if you can't get the DS3025 copy anymore then you hope for the best and wait if they are happy with the original
one that was sealed in the AOS packet. No need to write any note or submit any form at this point. If you get a newbie adjudicator or they lost the original
DS3025 then you would get a RFE and have to see the CS, at this point stop stressing, others have not gotten a copy of the DS3025 and been approved.
