Help with I-129F form Part 2

Hello all, 

 

I am hoping someone can help me with a question about the I-129F forms... I need to expand on both the address and employment history because I've lived and worked in more than two places in the last 5 years. I am thinking of just attaching a separate sheet instead of using 'Part. 8 Additional Information' at the back. I know what to put for page (5) and part (2) but what do I put for item number?? Has anyone had to expand on this part?
