HELP!! CIVIL SURGEON IN BROOKLYN, NEW YORK

Hi guys,

 

Does anyone know a civil surgeon in Brooklyn or Manhattan that I can go to to get my vaccinations record transcribed?

I did all my vaccines after my medical exam in London and now I just need a doctor to transcribe them and fill the form to send for my AOS.

I called a clinic but they told me I should get my medical done by them, which is weird cause I know a lot of you managed to only get the documents they needed without going through the medical exam again.

 

I would really appreciate your help!

As a K1 you are NOT required to send in a i-693 with your AOS if your DS-3025 is marked COMPLETE, just send along a copy of this if you have it. If you don't have it maybe the doc's office can fax you a copy.

 

The original DS-3025 is included in the file you gave CPB at POE and will be linked to your A-file, but it is good to show the USCIS that you have a copy of having met the requirements so that it is clear that this info is in their files.
