If all goes well, my fiance should have her visa in about a month. We plan on backpacking around Asia for a few months before entering the US. I know she has to enter within 6 months from the date of her medical. Is there anything else we should be aware of? Any risk in waiting to enter?

No. :) Just keep that passport safe and make sure to use the visa before the expiry date that'll be printed on it.

And keep that brown envelope from the embassy in a very safe place if you are going to be on the move a lot. It cannot get opened or torn. Guard it with your life! Buy a sturdy document wallet to keep it in.
