Dr writing me a letter for medical k1 anxiety

Hey all.

I'm quite concerned that at my medical my past history of depression and anxiety will come up - I presume they will need to be disclosed.

As an extra precaution I want to attend my medical with a letter from my dr explaining this history is not affecting me today; I'm not on any medication and successfully completed a course of CBT/am feeling a lot more positive etc.

Can anyone provide any pointers or key phrases/points I MUST include?

Depression Knightsbridge from Sunya and Tim

My fiance indicated he had battled some depression 3 years ago while going thru the separation from his wife. He took with him a letter from his doctor but Knightsbridge asked for this additional information. Just thought I would share so that you know all they will be asking. Hope this helps. He was given a form by the doctor and asked to have his doctor provide the following information. Please provide a brief medical report outlining:

• Diagnosis
• Investigations - if any
• Treatment
• Duration of treatments and illness
• Any prior or subsequent mental health illness
• Any incidents of self-harm, harm to others or suicidal attempts
• Prognosis and your opinion of current mental state

I followed these instructions exactly. My medical was last week, I got a letter from my GP saying all the key things, took the letter with me, Knightsbridge took a copy and the doctor didn't really even discuss it with me, just asked when was the last time I was taking any medication. So, follow the instructions and try not to worry too much!
