I am us citizen filing concurrently ( i 130 , i 485 ) for my parents who are in US right now on Visitor B2 visa.

Following is the cover letter containing the list of forms and corresponding supporting documents

Can somebody please confirm my list of documents and supporting documents ?

Please let me know if I am missing anything. Thanks in advance.

Dear Sir/ Madam,

Please find enclosed concurrent filing of forms:

G-1145 E-notification of application/petition acceptance,

I-130 (Petition for Alien Relative),

I-485 (Application to Register Permanent Residence or Adjust Status),

I-864 (Affidavit of Support Under Section 213A of the INA),

G-325A (Biographic Information),

I-693 (Report of Medical Examination and Vaccination Record),

I-131 Application for Travel Document.


Petitioner (U.S. Citizen): Petitioner name

Beneficiary (Mother/Father): Beneficiary name


Contents include:

1. Form G-1145 E-Notification of application/petition acceptance


2. I-130 Petition for Alien Relative

  • Check payable to U.S. Department of Homeland Security for $420 filing fee
  • Copy of Naturalization Certificate (Petitioner)
  • Copy of petitioner’s birth certificate showing the names of both parents
  • Copy of Petitioner parents’ marriage certificate establishing that petitioner’s father was married to petitioner’s mother before petitioner was born.

3. I-485 Application to Register Permanent Residence Or Adjust Status

  • Check payable to U.S. Department of Homeland Security for $1070 ($985 I-485 filing fee + $85 biometric fee)
  • Copy of beneficiary I-94 form
  • Copy of beneficiary current passport (biographical page ,visa as well as entry stamps)
  • Copy of beneficiary non availability of birth certificate and ????
  • Two passport-style color photos of beneficiary

  • I-864 Affidavit of Support (sponsor)

· most recent checking account statement

· payroll information

· Copy of Tax transcript for 2014,2015,2013 (filled jointly with wife)

  • Form G-325A, biographic information, for beneficiary
  • Sealed envelope containing form I-693, Report of Medical Examination and Vaccination Record


5. I-131 Application for Travel Document

  • Copy of passport page with nonimmigrant visa
  • 2 passport photos of beneficiary (attached with a paper clip to Form I-131 and signed)

Would you please post the results of your application. I am in exactly the same position as you. I am about to do a concurrent filing for my father who is here in th e US on a B-2 visa. Would like to know wether your list as posted was complete or did they request more items. If you could review my list and advise me it would be much apppreciated. It was posted under my name a few days ago in this same section.
