Affidavit of Support - Two jobs

Timeline

I am the USC and have started the K-1 visa process. I have been reviewing what will be needed for the Affidavit of Support during the next part of the process. I currently have a full time job and part time job. I have worked at my full time job for almost 13 years and the salary there is more than enough to qualify without a co-sponsor. However, I also have a part-time job that I have had for almost 4 years. There is not space on the affidavit for two employers, but my tendency is to want to provide complete information during this process. Should I complete a second affidavit or just complete a signed and dated supplemental page to go with the affidavit? Does anybody here have experience with providing information with the affidavit for more than one employer?

Liam's second visit to US: May/June 2015

Liam's third visit to US: Jan/Feb 2016

Got officially engaged!!!: February 26, 2016 (L)

Mailed I-129F packet: 3/7/2016

Received at Dallas Lockbox: 3/10/2016

NOA1 Text and Email Received: 3/15/2016

NOA1 Hard Copy Received in mail: 3/19/2016

NOA2 Approval date: 6/15/2016

Case Number from NVC: 7/15/2016

Arrived at Consulate/Confirmation (Packet 3) Email received: 7/28/2016

Medical Complete: 8/8/2016

DS-16o Complete: 8/12/2016

Email to Consulate Requesting Interview Appointment: 8/15/2016

Interview appt/Packet 4 Received: 8/22/2016

Interview Date: 9/27/2016 (FINALLY!)

You can use an attachment sheet to list the second job info, but I believe there should be enough room on the form to hand write in the info for both jobs, side by side or on top of each other. List the combined income amount. Provide documents from both jobs, such as employment letters and/or recent pay stubs.
