Send a copy to show you had all the required vaccinations. It is just kind of a reminder/flag to the adjudicator that you met the vaccination requirement for AOS.

Ok so I read the link that was provided and I have everything except for the MMR it says u need 2 checks or a code b.. Mine just has MMR checked and the date. Do I need to do this vaccine?

You are only required to get one shot in a series for AOS, so I would send it 'as is' an not worry about seeing a civil surgeon unless instructed later to do so.

Hi, I requested a copy of DS-3025 from the doctor in the Czech Republic. He sent it to me. Now I'm thinking if I should still go and have the I-693 done. I sent them the copy and the civil surgeon said that some vaccines are missing and they want to do:

- A 2nd MMR vaccine

- Influenza vaccine

- RPR/VDRL bloodwork

- TB skin test or Tspot.

Czech Republic has all vaccines though and for example influenza is voluntarily. I don't understand why they need to do that. I've already passed for K1 with my vaccines. Plus it's like additional $300.

So now I really don't know if I should go or send AOS just with the DS-3025.

I think a lot of it depends on what immigration officer reviews the file. For instance, we sent in the DS 3025 (which we checked to make sure everything was filled out or waived) in the initial packet for AOS and hadn't heard anything for 5 months then last week we received an RFE for the vaccination supplement of the I-693 done by a civil surgeon. We found a civil surgeon that only charged about $25 to transcribe the vaccines and they made sure everything was filled out and sealed. So for some people the DS 3025 works but in some cases they really do want to see that I-693 completed by a civil surgeon.

K1 Visa

January 13, 2015: Mailed I-129f packet to Dallas Lockbox

January 15, 2015: I-129f packet was delivered

January 21, 2015: NOA1

March 23, 2015: NOA2

April 8, 2015: Left NVC

April 12, 2015: Received by Consulate

May 1, 2015: Packet 3 Received

May 13, 2015: Interview and Approved!

May 19, 2015: Visa Received!

June 30, 2015: She arrived in the USA!

AOS, EAD, and AP

September 23, 2015: Mailed AOS, EAD, and AP packet to Chicago Lockbox

September 29, 2015: Received NOA1 for AOS, EAD, and AP

October 21, 2015: Biometrics Appointment

Flu shot is required during flu season, Oct. - Mar.

You do not need a new medical or any parts of one, such as blood work or another TB test. Find another civil surgeon.

You can also send your DS-3025 as it is, and only find another civil surgeon if you are asked to at some point.

Hi Vj's . I got in the US September 2016 got married November 18 and was able to sent the AOS last December 19. I'm just wondering if I did the right way.in my Ds 3025 it says in Summary that I completed the vaccination requirements. but I just also did read that I need 2 MMR vaccination but I just had 1 . I got confused with this? If I still need another shot of MMR why in the summary says completed?
