IMMUNIZATION RECORDS

1. I know I'm going to need an immunization record eventually, but my Dad contacted my doctor and he said they had none since the last shot I was given by them was in 1994. My doctor does know which shots I had though apparently, so he said he would write them down, but since there is no record, I guess he doesn't know the exact dates.

2. I got a Tetanus shot about 3 years ago at a clinic because I stepped on a piece of glass. The clinic has moved (it was an Appletree one.) Would they (I guess I'd have to try figuring out where the new location is) have a record of this one shot?

3. My Dad said that he found out (or heard, or something) that the government keeps a record of immunizations. Is this true, and if so, how can I obtain that?

4. If it's not possible to get one or any of these records, what do I bring with me to my medical examation and especially interview? Am I going to get a new record after the medical exam?

5. Is it worth it to get new shots before the medical examination, and if so which shots should I get of the ones I will hopefully be able to figure out that I don't have? My Dad said something about the Influenza one not making sense to get in the summer... (I think)

THANKS.

Ask your doctor to do antibody titres for you so at least you have a "date" to accompany the proof that you are immune. They don't need to know exactly when they were done, just evidence of childhood immunizations for Tetanus Diphtheria and evidence that you are immune to the others. Antibody titres are covered under OHIP.

These are the following age-appropriate ones 18-64: http://www.panelphysicians.com/Pages/Age6.htm

Public health departments are supposed to keep records on hand, but they didn't have mine.

Hi i'm the canadian posting this time, I think the local health unit would have your records since they need to have all your information from the first day you started school at the age of 4 or 5 and they usually keep it on file may take them abit to find your file if they havent transfered it all to computers yet but would think they should have it since they are pretty fast to send me notices to when my kids get their shots and i dont send them a copy right away.

I went to my local Health Unit, they no longer had my records.

I just went to the Medicentre and told the Doctor I needed to get a Titres test. He sent me to the Lab to get one. They will check for Measles, Mumps, Rubella and Tetnus. My results came back and I showed low on a few things. I went back to health unit and had all my shots redone. I went for another Titres a few days ago and am waiting for the new results.

When you go for your Medical the Doctor will take Oral Testimony if you have had Chicken Pox.

If you are unsure where your records would be, you can always try phoning the Junior High School you went to. Did you get your shots at school? They will know who does the shots and where they go.

i was able to get mine from my local health unit. however, i was told that the only reason they had them on file was because my parents submitted them to the health unit 'back in the day'. had they not submitted them, i would have NOT been on file. i think it was strongly suggested to submit them .. but not manditory.

that place should have a record of that one shot you recieved a few years ago ... ...

THANK YOU! I did some googling and came up with http://ottawa.ca/residents/health/conditio...n/index_en.html and called the number on there. Unfortunately I was calling from Skype and I don't think the reception was completely clear so I felt like I was yelling at a couple of points (lol! poor lady) but I was able to get across my birthdate and most of my name (she just thought I said ''r'' instead of ''l'' or something like that) and then she told me they had a partial record and asked me where I wanted her to send it to me. So at least that's something, and hopefully even though she says it's just a partial record, it's still most of the shots I got (esp. since the last one was in 1994!) Anyway, thanks!

I had my doc do an antibody titres test but the lab messed up the results and tested only for Rubella and not MMR and Td ... damn lab. My medical was scheduled in 4 days so I could not wait another 2 weeks to do another test I ended up just getting the shots which cost $60 all in all and it was given in 2 days with a record given from the doctors office.

My husband just went to a walk-in clinic last night, they gave him the MMR and TD, he was done in less than 1/2 hour and it was free :) They gave him an immunization record on the spot.

We were going to get the titres done, but he waited so long, and I've heard of people not even having their immunity show up on the titre, so it was easier to just go and get it done in one shot.

So I just called Public Health here in Saskatchewan and I now have an appt to get my shots done for FREE!!! Now this is the ONE good thing that has happed lately. I'm still waiting on my passport........... :( so of course that is my biggest worry. I guess I just don't get why the Medical Doctor needs a passport...I'm still in Canada for goodness sakes.

Just because they are mandatory does not mean you had them done. (Which is why US immigration wants proof.) From what I remember, you are to have two rounds of MMR shots when you are a child. I only had record of having the first round done. I got titres done showing that I was immune to Measles and Mumps (I had chicken pox as a child) - but not Rubella - so I had to have an MMR shot anyway.

I would go to a walk-in clinic, tell them you have no record of ever having your MMR shot and that you need it ASAP (along with proof that you've had it done) because you are immigrating to the US and they require it. Explain that you cannot wait "weeks" for the titres/results to come back. I don't think there is any harm in having this shot done (even if you are already immune) since, when I had it done, I was already immune to Measles and Mumps.
