HELP Do we need I-639 filled out by a CS???

Hello my wife and I are a little confused we don't know if she still have to go to a CS

Is her DS-3025 complete or do we really need to go to a CS

which I heard is very expensive please help us

I've attached a copy of her DS-3025

There is no 100% answer for this. Many people submit the DS-3025 and it is accepted. If it is not accepted, then yes you will need to get another I-693 from a civil surgeon.

Your DS3025 is not marked 'complete/blanket waiver' so yes you have to see a CS or you can wait to see if you get an RFE.

NVC Received : some dinkelsberry yehoo in the house of clingons send our petition to the wrong consulate.

Consulate Received : July 30,2014 Transfer to right embassy complete.

Interview Date : Oct 22, 2014

Interview Result : AP , requesting another PC (not expired) and certified divorce decree (was submitted)Stokes interview via phone for petitioner 4 hrs after interview.

Oct 23 email notification visa approved.
Visa Received : Nov. 3 , 2014 VISA IN HAND.

US Entry : Nov. 21, 2014

Marriage : Dec 27, 2014

AOS send : May 12, 2015, received May 14, 2015 USPS priority

Email &text : May 18, 2015, check cashed May 19,2015, return receipt May 21, 2015 stamped USCIS Lockbox, NOA1 (3x) May 22,2015

Biometrics : June 1, 2015 letter received for appointment June 8, 2015, successful walk-in June 1, 2015

RFE : June 12, 2015 for income not meeting guideline. Income does ( ! ) exceed guideline.

RFE response : June 26, 2015 returned with a boat load full of financial evidence.

UPDATE: July 5, 2015 updated on all 3 cases, RFE received June 30, 2015.

Service request : Aug 12, 2015, letter received that it will be processed within 90 days from receipt of RFE.

UPDATE: Aug 24, 2015, EAD card being produced/ordered. ( 102 days from AOS receipt day and 55 days from RFE response received.) Thank you Jesus !

July 21, 2017 HOME VISIT.  Went well. Topic thread in AOS forum.
Waiting to skip ROC and get 10 yr GC due to over 2 year while pending AOS
AOS APPROVED Oct. 4, 2017 * Green card in hand Oct 13, 2017 !!!!!

First K1 denied after 16 month of AP. Refiled. We are a couple since 2009. Not a sprint but a matter of endurance.

My wife and I went to a CS. She has the same exact vaccines your wife has and they made us get a 2nd shot of MMR.

The CS we went to were willing to just do the vaccine transcription for i-693 form. He charged us $60. If i were you, I'd check out the huge thread on here about CS that wont rip you off, or start calling around your area and see.

The CS we went to were willing to just do the vaccine transcription for i-693 form. He charged us $60. If i were you, I'd check out the huge thread on here about CS that wont rip you off, or start calling around your area and see.

is there a CS that is hired by USCIS or you can see any CS?

It can be $50 - $200+ according to the doctors around my area. My asawa got the same as yours and I'm going to take my chance and submit as is. If we get RFE then I'll go to CS.

hello? do i need to see a CS first and then file for the AOS or file and wait for the RFE? what is the best thing to do.. hope you can help me.. thanks

A Civil surgeon is a doctor, selected by the USCIS to conduct medical examinations of aliens in the United States who are applying for adjustment of status to permanent residence, or who are required by the USCIS to have a medical examination.

You can see any CS.

Hi. I got the same concern.

My ds3025 meets correctly as what it was stated here http://www.visajourney.com/forums/topic/551080-immunizations-ds-3025-or-i-693/

Except Influenza (mine mark D instead of F).
