yer supposed to use the one in the payment portal. it's downloadable and printable.

Sometimes my language usage seems confusing - please feel free to 'read it twice', just in case !

Hi. I was confused too. I printed the one they emailed me and sent it.when you call they will tell u different things but I sent in the affidavit with my tax docs etc and when my husband police report was ready which was about three weeks later, I sent a second mail. So if u don't have everything u can send in the affidavit with your other financials and send the rest as soon as you can.

We used the Document Cover Sheet that came with the welcome letter, and got case complete after 3 weeks.

The separate barcode sheet will have the same barcode as the document cover sheet, so you can use that.

Maybe put the documents in the same order that the document cover sheet advises though...?
