I finally got my immunization date after 2months of waiting! I called St.lukes today OCt28 as they scheduled me to call, but its seems forever to reached them. No operator answered after spending lots of pesos. (i used cellphone) So i decided to email them at slec@slec.ph and finally got the answers. On October 30, 2014 for my immunization and release of my medical examination result..YES!! SO okey my question is this. For my interview preparation should i get the original TAX and W2 forms?or it can be scan? I have here the original Letter of intent and my faincee I-134 form with the original signature.

Any help?

So happy for you sis for the negative result. Did they let you choose the immunization sched? For ITR and W2, scan copies are acceptable as I've read from others.

- COPIES of the most recent tax return and W-2, so those can be sent as attachments in an email if wanted.

k. EVIDENCE OF SUPPORT. You should be able to demonstrate that you will not become a public charge
or be a burden on the U.S. taxpayers for financial support. A completed I-134 Affidavit of Support Form
with original signature will be useful to the consular officer to evaluate your petitioner’s ability to be
financially responsible for you. You should also submit your petitioner’s most recent U.S. Federal income
tax return (Form 1040) and wage statements (Form W-2). Employment letters stating salaries and bank
statements may be included to substantiate the I-134. Download Form I-134 at

I sent my wife copies of my ITR and W2. Went just fine. I also doubled down and went to the IRS website to download copies of my tax transcripts, printed them out, sent them to her to.

No originals needed. Copies are fine. But make sure the 1040 is supported by the W2. 1040 alone will not fly because too easy to get a blank form and fill in your own numbers.

And make sure the copy is a copy of the signed original.
