Are you applying in the Philippines or outside Philippines.

If in Philippines, you can apply to any provincial office. But if outside, I believe the request has to be in Manila.

I applied here in UAE and the Philippine Embassy told me to send the request to Manila.

Just make sure its the green form with the notion no record on file or you will have to get a letter of explanation later at the Embassy stage good luck on your journey
