Form I-134, Affidavit of Support questions

Hi.. I am in process of filling out my Form I-134, Affidavit of Support

I have a question in regards to question no.9

9. I have previously submitted affidavit(s) of support for the following person(s). If none, state "None".

there is only 2 lines... but if I supported previously 4 people in the past 4 years . how do I make room to add the 2 additional person ?

so basically attach and blank sheet and write additional information for question 9 on the top ?

also I am assuming I will write my name and dob and Form I-134 on top ?

The instructions on filling out forms says

Don't forget to note "see attachment" on the space provided under question 9

You're status states AOS, the I_134 isn't used for AOS
