Filling out I-693 (Report of Medical Examination and Vaccination Record)

Hey,

I've just filled out my I-693 and I'm wondering, when I take it to the Civil Surgeon does he need me to provide proof of the medical exam I had in Knightsbridge, London?

I have proof of my vaccinations but I can't seem to find the proof of the medical I had. However since I needed to have a medical to even apply for a K-1 VISA I'm hoping that I don't have to provide that to the civil surgeon?

Is there anything else I need to bring to them?

Are you missing vaccinations? Some don't need to go to a civil surgeon.

Did you ask what the Civil Surgeon charges?

You don't fill out I-693. That's not for you to do.
