I am the beneficiary. Again I needed help here, we are going to fill-up our I-134 form.

I hope you don't mind if I ask item by item, just want to make sure that we file it correctly

and I needed check our answers.

My fiance earned income annually $ 35,000 and I think enough for the two (2) persons as I read the income guidelines to petition without having co-sponsor.

He was born in the states, so on

item# 1. Do we leave all blank from a.~e. ? or do we need to write NONE on each item?

item#3. We put my information (beneficiary), I am single and no children, so we leave blank again for the name of spouse and children below. Is it correct?

item#7. He is a delivery driver, we'd practiced to fill-up the form, we put "Transportation" as the type of business,

but we read the sample form here, on the sample it showed "Supervisor", so now we're thinking maybe that question is asking the position,

We got confused here.

And for the following question about income, assets, etc. We only wrote the annual income and leave blank the rest...Is it correct?

item#8. My fiance (USC) still giving alimony to his ex-wife every week,

Do we need put his ex-wife name here?

item#9. We put NONE.

item#10. On the sample form it is for the beneficiary. Is it correct?

item#11. We dont know what to put here...Do we need to check what box?

and what is the comment for the space below on that item?

Lastly on Oath or Affirmation of Sponsor, there is a blank space after that statement...as amended___________what is this for?

Do we need put his ex-wife name here? <-- If he does not claim her on his taxes as a dependent, then no.

item#9. We put NONE.

item#10. On the sample form it is for the beneficiary. Is it correct? <-- Yes.

item#11. We dont know what to put here...Do we need to check what box? <-- Just like on the example form.

and what is the comment for the space below on that item?

Lastly on Oath or Affirmation of Sponsor, there is a blank space after that statement...as amended___________what is this for? <-- If you mouse over the data entry box for the 'Oath' section, it tells you what it is about and instructs you to enter the sponsor's full name.

Yes leave a-e blank.

#3 ) Yes only list you (beneficary).

#7 ) Put transportation, or whatever. The embassy only cares about the tax return and W-2 info. You don't need to have bank account info, assets info. As for alimony it has nothing with anything... unless there are children from the previous married as they need to be listed.

#9 None works.

#10 putting your name (beneficiary) there is fine

#11 Really doesn't matter. In the space below simple write: K-1 visa.

Thing to realize is the I-134 is not designed for the K-1 visa, so some questions really don't fit/work/or make sense.

Ignore the blank space... I did. ;)

There are two people, sponsor and beneficary (fiance and fiancee...unless there are children)

You would do better to post your questions relating to USEM on the Philippines forum.

I have follow-up question related to item#10, we wrote my name (beneficiary) and noted it " Now filing with I-129F Petition".

My question is for the Date submitted, Is it the date when we sent the petition?

Use your NOA1 receipt date.
