Hey Everyone

I need to create a supplement sheet for my G325a. I have a few questions regarding this.

1. On the G325a should I use the first 4 rows and then put see additional sheet in row 5. Or should I just put see additional sheet in the box and put everything in the supplement sheet. Or does it not matter? I think I would prefer to put everything in the additional sheet so that I can fit in all of the addresses for everywhere I've worked

2. I can't see anywhere if there is a specific format I need to use for the supplement sheet. Does there need to be one? Or can I just make my own as long as it has all of the required information?

3. I have attached the additional sheet that I've created. Could anyone tell me if it looks ok?

1. You can simply put "Please see attachment", and write all the answers on the additional sheet.

2. No specific format. As long as it clearly shows which form it supplements and the question/s to which the answers correspond to.

3. Format looks good. Btw, I don't think you need to say you're unemployed for a particular period. It only says to answer "None" if the applicant is never employed at all. :)

Your supplement sheet looks good. I would list everything on your supplement sheet, and on the G-325A form just write SEE SUPPLEMENT.

I would do the form exactly as you have.

I've attached my G325a. Does this look ok with how I've explained it and put all of the details etc?

Bill, while I have removed the copy of your completed form for your personal security, since you are worried, I will say that it looks good and should be fine to include in with your submission.

In the future, if you wish to post any of the documents or forms, please take the time to remove the personally sensitive information from the form - you can make a photocopy of the original and then black out things like full names, birth dates, addresses, case and file numbers, ssn, etc. and then copy that for the site. No one here needs access to information like that, and since you don't have to be a member to view Visa Journey, posting this information here becomes a serious identity fraud risk.
