Its been 2hours now, im tired calling the civil surgeons and I cant find anyone that is not a rip off. They are charging 300-500dollars. That is just insane. Does anyone in Michigan that can help me? And know a civil surgeon that wont charge that much?

I already answered your question on the other post. If your vaccination records is completed or the blanket waiver eligibility is ticked, you don't need an I-693.

My vaccination history incomple was checked too as well as applicant may be eligible for blanket waivers. So is it as good? So you dis not file the I163 in your AOS when you passes it to the embassy?

Yes, it is good. I only sent a copy of my DS-3025 with my AoS papers.

No, since you were not given the Tdap vaccine, you will need to get that shot and get the vaccination records transcribed to the I-693. Check with your local county health clinic to see if you can get the shot cheaper than at the civil surgeon's.

Form I-485 with supporting documents: 2 passport-sized photographs, a copy of birth certificate with English translation, a certified copy of marriage certificate, a photocopy of Indonesian passport biographical page, a photocopy of Indonesian passport K-1 visa page, a photocopy of I-797 Notice of Action of I-129F approval, a copy of I-94 arrival/departure record, and a photocopy of DS-3025 Indonesian medical exam along with I-693 instruction for K-1 visa holder.

Form I-864 with supporting documents: a copy of 2013 federal tax return and W-2.

Form I-765 with supporting documents: 2 passport-sized photographs, a photocopy of Indonesian passport biographical page, a photocopy of Indonesian passport K-1 page, and a copy of I-94 arrival/departure record.

Copies of documents are exact photocopies of unaltered documents and I understand I may be required to submit original documents to an immigrant or consular officer at a later date.
