Evidence of Support Quetions

Hello,

I have a question about the proof of support. I'm currently 30 years old, and moved back in with my parents. I was working as a Nuclear Medicine Technologist at a hospital in New York City. I was laid off about 2-3 years ago, and ever since then have been working per-diem nuclear tech jobs, and some temp full time positions, and collecting unemployment in between.

The reason for that is because I decided to go back to school and take pre-requisite courses for Physician Assistant Masters programs. I was taking really difficult science courses (chem, bio, micro bio, organic chem, anatomy and physiology) each semester that I needed to get all A's in (which I did). Now I'm currently waiting on applications to Physician Assistant Programs which I should hear from by June 1st.

Being that income was about 20-40k/year the last 3 years (it varied greatly depending on how much I got called in at the hospital), which included unemployment, will I be considered unable to support if I don't ask my parents to co-sponsor?

So I have a bachelors in Health Science, I'm licensed to practice Nuclear Medicine in the State of New York, and I have about $15,000 in my bank account. I have no other significant debt other than my undergrad school loans which I pay.

Will I require a co-sponsor? Does the Ukrainian Embassy even accept co-sponsors?

So then do I submit a I-134 for my father and myself or just my father?

Which includes W-2's, pay stubs, & bank statements. Anything else?
