Yes, you need to go to a civil surgeon here, to get i693. Do you still have a copy of your vaccination sheet from the overseas medical? you could take that to the doctor here. i only had to take here the Tetanus and flu vaccines, but as the doctor explained it, he did test my blood to see if i have other vaccines, like MMR and others.

here is the link with the civil surgeons accepted by USCIS: https://egov.uscis.gov/crisgwi/go?action=offices.type&OfficeLocator.office_type=CIV

Well, I think these two(tetanus and flu) you might have to take them, i'm not sure. The 2 vaccinations cost me 100$, but i had to redo my whole medical here, so per total, it cost me 415$(that includes TB skin test, blood tests, the vaccines and the consultation). See if you can find a doctor who is willing to do only your vaccinations, not the whole medical exam. If your overseas medical exam is done within 1 year, you are not required to take the medical exam again, but some doctors will rip you off by telling you they wont accept to do your vaccinations unless you do the whole medical exam again.

You do not need a new medical if yours was done within a year of filing. You are not required to get a new medical if the processing of your case goes past the year mark for your medical. Also, it is not flu season, so you would not be required to get a flu shot when going to a civil surgeon to get your missing vaccinations and vaccination transcription onto an I-693.

Your DS-3025 needed to be completed like this, or you needed to see a civil surgeon > http://www.visajourney.com/forums/topic/428381-i-693-report-of-medical-examination-and-vaccination-report/?p=6206232

The flu shot is often only applied during flu season, and not sure it really is mandatory; when my now wife came over it was September, about to get in flu season and it was not applied. For other vaccines, you might want to research for civil surgeons approved by USCIS; it seems there is no rhyme or reason to what they charge, even in same city. You can do it in another state as well; when we did ours, it was in Denver (tough we lived in Houston), we just had planned to marry in Colorado and as I was researching, found a surgeon in Denver that had very low fees at a fraction of what they wanted in Houston, so we did it there.

I found links here in VJ that had posts of many members listing surgeons. They were old but useful.

Also, watch out for those who want to do a full physical and charge you an arm and a leg. Full physical is not needed, but they will want to convince you otherwise.

I just did my medical on tues and got charged $120 for transcribing only. $50 for tetanus. That's the only shot I am missing. I showed doctor my birth card and DS-3025
