AOS recent documents

Hi All,

I am looking for the recent list of documents to submit for the process of AOS. Do I need medical examination again for AOS??

You include the copy of the DS-3025 you got from your original medical exam. You don't need a new exam unless it has been more than a year since your last one, or if the vaccination history on the DS-3025 was marked incomplete.

thanks for your reply. I have the copy of DS-230 and the doctor mentioned two things in the results.

1) Vaccine history incomplete

2) Applicant my be eligible for blanket waiver(s) because vaccinations not medically appropriate (as include above)

Am I eligible for waiver or my vaccines are incomplete???
