Hi all....my wife is from Peru and we submitted I-485 in May 2012....just got notice that we failed to submit proper immunization records on form I-693.

I went over every detail and she had her medical exam/immunizations before leaving Peru by a US approved doctor.

Anyway....we got the vaccinations done by civil surgeon.....my question is, do we mail to to the address we sent our I-485 to adjust status, or do we mail

it to the address on the envelope when we got the notice that form I-693 was needed? Our case is being handled in Missouri, but the notice came from

Raleigh, North Carolina. Which address do we mail the form to? Thanks!

They really should include something more noticeable about the I-693 in the guide. We found out only a day before we were going to submit our EAD/AOS package, and by that time the most available help to take her to a civil surgeon to get the missing Tetanus booster were on the other side of the country. I've had to wait to take time off from work to get this done, which has delayed us by weeks.

This is an item in the VJ Guides for the 485 form as part of the AOS Guides. Don't know what more notice you need?

11. I-693, Medical Examination of Aliens Seeking Adjustment of Status.I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. If you are missing any shots from your overseas medical a I-693 vaccination transcription is required to be completed by seeing a Civil Surgeon to have certain portions of the I-693 completed and have the Civil Surgeon certify the form (sealing the original in an envelope and giving you a copy).

She had her examination before coming to the US (including immunization) which we did submit before she even received her K-1. In anycase, we got it done, and my question is, do we send it to the center handling our case (which is in Missouri) or do we send it to the address which is on the envelope when we received the notice (notice came from North Carolina, US Department of Homeland Security)?

Send to the address for the RFE. If the only address for the RFE is on the envelope, then send it to that one. Seems like there should be some address on the RFE letter though. Sounds like the RFE came from your local USCIS office and not from the NBC.
