You get the 3025 form from the medical facility where you took your medical before the interview.

Keep lolling for more CSs or plan to spend the money.

I have a copy from the embassy appointed civil surgeon that stated I am in good health and also a copy of my vaccination records. You should get all of those.

I got a copy of the vaccination record but just going in to see a surgeon is $150 so who do I show it to prove that am in good health?

The doctor should give you a health certificate stating that you're in good health. I have one for that. Are you sure not lost the paper?

Edit:

I guess those papers need to send for AOS. I might be wrong about this.

You just have to fork the money to have the transcript done- it is part of the process none of us like it but you have to do it for AOS. Just the vaccines not a medical exam so you need to find a CS who will do the transcribing for you

You dont need a copy of the medical exam. We didn't have one from Accra house and many others do not have them from the medical before k-1 interview.

They just want proof of the vaccines and the only way to do this is going to an CS for transcript
