I am beneficiary, we are gathering all required documents for I-485 and I noticed that my DS-3025 says page 1 of 2 but I have actually only one copy from St. Lukes after my medical last May 24, 2013. I am worried that they missed to give me page 2 of it and out of my mind I never checked when I was in Philippines.

Like many other I will only attach the photocopy of VACCINATION DOCUMENTATION WORKSHEET I hope this will be fine to USCIS without form I-693.

I need your opinion to this matter, thank you so much for the time.

Here is what I suggest for you to do:

1. DS-3025 is only one page. Page 2 is just an instruction. You don't need it. :-)

2. You MUST fill up Form I-693 (only page 1 only)

3. You need to find an authorized local civil surgeon to have your vaccination record transcribe to Form I-693 (page 5)

*note: when going to the civil surgeon bring your passport, DS-3025 original and copy, page 1 of I-693. They normally charge from $50-$75 for transcribing the documents. If they asked you to go through another medical exam just tell them you don't need it.

I forgot, they will give you the page 1 & 5 sealed in an envelope. This is what you will need to submit for AOS. They should provide you a copy of what is inside the envelope.

You'll be fine as in my case they also just gave the page 1 of 2 for DS 3025. I had my medical June 2013 and filed my AOS October 2013. I only sent my DS3025 and did not get I693. As what you said, you are still within the 1 year limit. I did not get any RFE and I'm already scheduled for an AOS interview this January 2014.

I just wrote in my cover letter table of contents beside the DS 3025 this

"DS-3025 (Copy of vaccination supplement according to 7.C. of Initial Evidence in the I-485 instructions)"

2. You MUST fill up Form I-693 (only page 1 only)

It is not a MUST to see a civil surgeon for the I-693. If you had all the required vaccinations at the time of your K-1 medical and your DS-3025 is filled out correctly, then you can just send a copy of your DS-3025. If you are missing vaccinations and/or your DS-3025 is not filled out properly, then you need to see a civil surgeon.

Fiancé(e)s: If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your Form I-485.

c. (2) Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form I-693.
