So my fiance did his medical in Islamabad and they did the necessary tests and gave him no vaccinations at all and told him vaccinations were not necessary. He had his interview and handed them the packet given by the physican to the US Embassy. My fiance then got vaccinations done from another physican in Pakistan after getting his visa.

So now that my fiance is here in the United States and we have had our court marriage, I am filling out all the forms and I am confused on if I should fill out the I-693 and have my fiance go to another physician here or will the documents of vaccination work from the physician in Pakistan?

I read somewhere that the physician has to fill out this form and this form was not filled out in Pakistan and also the packet given by the first physician is already given to the US Embassy so we don't have those either. I am a little confused so if anyone knows please help.

You need to see a Civil Surgeon in the US, They need to transcribe the vaccinations onto an I-693.
