what are some reasons for visa being denied?

i was in vietnam for the last 4 months

i met a girl and i intend to marry her

we lived together for 3 months

is there a problem since i've only known her a short time?

also, i dont have any income in us

should i wait til i've known her for a while and/or have a steady income in the us?

No income in US is going to be a big red flag

If you can find a co-sponsor! It is not a red flag! Its just that you will need to find a co-sponsor

Do you have enough evidence? Hotel receipt, passport stamps, boarding pass?

From filling to interview takes like 5 months so you can build evidence like chat logs and phone calls.. I think you will be fine!

This here sounds like it'll be your biggest hurdle. You need to show that you can support her without receiving government handouts.

Good sense is to wait a bit and get to know her more as well.

Do you intend to go back to the US soon? If so, how are you going to support yourself and her?

If you plan on staying in Vietnam for a while yet, it might be better to marry and apply for a spousal visa.

From what I know, reasons for denial are usually either for failure to meet petition requirements (like a couple who hasn't met in person in the 2 years prior to filling the petition) or big red flags, like police or drug issues. For some high-fraud countries, where the embassy interviews tend to be stricter, there can also be problems with establishing a bona fide relationship or failure to meet income requirements. There are two steps where you can be denied, at the Petition phase, or at the Embassy phase. Embassy denials tend to be country-specific so you may want to do some research on here for Vietnam cases.
