Hello Friends

This site has been a life saver thus far and I am very grateful.

I am about to Submit my Adjustment of Status Package ( I came to the US on a K1-Fiance Visa and we got married here).

However, There is a minor Issue ( hopefully the last oneranting33va.gif )

When it comes to the I-693 Medical examination requirement, I no longer have that form, My examination and sealed form was submitted with my K1 package and US immigration now has possession of it.

However I did research this and it seems all I need to submit is my Vaccination documentation Worksheet filled out by the the Panel Physician ( DS-2053) .

Should I include the ORIGINAL or a COPY?

But I also still have a CD of My Chest x-rays, should I submit that as well?

what should I write on the checklist to include this? I wrote : Copy of DS-3025 Vaccination Documentation Worksheet

To be honest, I am extremely stressed out and in a bad mood!! These forms are driving me Crazy and I feel so dumb for being so lost. I keep on thinking , I am done but I stumble on something new that's wrong again!crying.gif crying.gif crying.gif

Also, I get Various versions of the required checklist online. Various Applicants post their checklists for the marriage based AOS online and each one looks different almost, People include sooo many differnet things !! I can't decide if its necessary or not!

we have the same case, I want to ask you too, did u submit all your forms for the adjustment of status I-485, employment authorization I-765 and the travel document I-131 already? if so, did you pay it one time together? and how much?? thank you..

Send a copy of ds3025 and write on the checklist you've sent it. No need to sent CD. See http://www.visajourney.com/forums/topic/101202-faqs-forms-i-693-and-ds-3025/ for more info.

Breath, don't stress out. Study the checklists and make your own mind up on what to include. There is definitely a pattern in all of them although they might vary slightly depending on cirucumstances etc. forms you need for sure are listed here http://www.visajourney.com/content/k1k3aos and some tips http://www.visajourney.com/content/k1aos
