I just got my MNL case number today. What do I do next.Can I schedule the medical/ interview right now(after payment of fees)?

Yup, you can schedule the interview and do the medical. Schedule the interview far enough out to allow time for the file to be shipped from NVC to USEM.

And one more thing Hank, my fiancee has kids, would I pay them separately and do the same for scheduling?

Separate visa fees yes. Not sure about interview times. I think I read that you must get interview dates for the kids, but the actual interview for them are done at the same time when your fiance has hers.

Yup a visa fee for EACH, and when your fiancee is scheduling for the interview near the end of her process it will ask if you want to schedule for another... that is where/when you do the scheduling for the children. (they interview time/date will be the same for all)
