Form DS3025 says Vaccine Incomplete, Blanket Waiver...

Hi Everybody,

I had my medical appointment and the doctor told me it was all good and gave me the sealed envelope.

On the DS3025 Form it says;

Vaccine History Incomplete, Applicant may be eligible for blanket waiver because vaccination not medically appropriate.

I don't understand. Should I worry about it? What does it mean?

Thanks a lot to whoever can help me and reassure me. I am having my interview in less then 24 hours.

Nothing to worry about, it's normal. It's not going to be a problem to get the K-1 (vaccinations are not required to get a K-1 because it is technically a non-immigrant visa), but it may be later for AOS.

Just check that your vaccination history seems complete (there are some explanation on the AOS forums by Nich-nick that are great). Basically, if you are not missing any vaccines (except some that are not age appropriate or insufficient time interval), you will be able to just send a copy of this form in your AOS paperwork.

Good luck for your interview! Relax, have your documents well organized and easy to find, write your address on the chronopost envelope and you'll be fine. :)

K-1 Visa Timeline AOS Timeline

- Aug 31st, 2011 - Mailed I-129F package - May 29th, 2012 - Mailed AOS/EAD/AP package

- Apr 13th, 2012 - Visa received - Aug 24th, 2012 - Green Card received

ROC Timeline

- May 19th, 2014 - Mailed ROC package to CSC

- Aug 8th, 2014 - Green Card received

N-400 Timeline

- Dec 29th, 2021 - Filed online. Got notice that biometrics will be reused.

- Now waiting...

Make sure you keep that paper as you will need to send a copy of it along with your AOS package.

If I may, I have a question concerning the vaccinations. By the time of vaccination, I am supposed to have all of them except one detail. As of now, I am missing "deuxieme rappel" for one vaccine and one which is not done completely. My family doctor says that we will be able to get the first vaccine up-to-date. For the second vaccine he said that I will need to wait at least one year to get "deuxieme rappel". Do you think it will be an issue if the medical panel doctor will see that one vaccine needs the second shot?

I am sure you will be fine. Based on what I have read on some blogs, we will surely get booster shots once we are in the USA and married.

The thing is that I am not french, it's been 5 years since i live in France. I am worried about medical history because I didn't need a doctor often. Yes, there were few visits but I cannot show the entire one. Actually I have one from my country, but it covers only primary school years (age 8-17 years) and is written in latvian.

My doctor wrote me a kind of "attestation" where he assures I have no regular treatment etc. I really hope this will not be an issue during my visit! What concerns the vaccination card, there everything should be in order.

... I forget you to ask : what were the necessary documents that the doctor wished to see absolutely? Are there any specific documents or evidence to bring?

Hi again xeon!

For the doctor appointment, you need to have:

- Passport.

- 1 Picture.

- Vaccination book.

- Letter from the US Paris Embassy with appointment date and time.

- Payment: cash or check.

06-11-2007 = NOA1 date (unknown to me).

07-20-2007 = Phoned Immigration Officer; got WAC#; where's NOA1?

09-25-2007 = Touch (first-ever).

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

12-18-2007 = Call; file is with Division 9 ofcr. (bckgrnd check); e-prompt to shake it; touch.

12-19-2007 = NOA2 by e-mail & web, dated 12-18-07 (187 days; 201 per VJ); in mail 12/24/07.

01-09-2008 = File from USCIS to NVC, 1-4-08; NVC creates file, 1/15/08; to consulate 1/16/08.

01-23-2008 = Consulate gets file; outdated Packet 4 mailed to fiancee 1/27/08; rec'd 3/3/08.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).
