Hi,

Today I found out from operator that i gotten a checklist in I864ez form . that i forget to sum up total member in the house hold.

so what should I write in the subject line in the email when responding checklist.

1. MTLxxxxxxx - AOS- responde to checklist or

2. MTLxxxxxxx- AOS-supervisor review

hi,

I 864ez form

no. 19.

I only filed 2012 tax return and have nt file 2010 and 2011 return.

should i check mark the first box or not?

because it says I have filed a Federal tax return for each of the three most recent tex years. I have attched the required photocopy of my federal tax return for only the most recent tax year.(In the form)

Please advice..

Hi,

You need to send another I-864EZ form for the correct total number of household. Don't email the form but if the first AOS sent via email then go ahead just email them the correct form with subject: Case no. - AOS checklist. In your 2nd question, yes just check the first box recent 2012 ITR. Hope it's help.

You can put the Case Number and Beneficiary's Full Name on the Subject Line.

On the email body that's where you put the "response to AOS checklist" then send the new i864ez as attachment file.
