Hello everyone. We are wondering what to do with the address page here. The information provided below is fake for example purposes.

My fiance's address for her house and school are very long.

Where do we write the "Attachment" that people talk about for long addresses or other info that won't fit on the computer form? What do we write on the "Attachment" page?

Basically I just want to know how to do it all. Please help.

Also, is the information correct? What do I put for city and province or state if it is Lima?? I am lost.

I was wondering about this too but I found the answer in the document Instructions for Form 129-F under General Instructions. 2. If extra space is needed to complete any item, attach a continuation sheet, indicate the item number, and date and sign each sheet. Since it states to date and sign each sheet, it seems like you should have a seperate continuation sheet for each item that needs more space. Since you are filing for fiance, I assume you are filing I-129F. The item number on my sheet for fiance address is in Part B number 2.

According to the CIA World Factbook, there are 25 Provinces in Peru. Lima is listed as a Province. I think you are correct in having Lima listed in both City and Province/State.

The form you are showing is G-325A. I cannot find special instructions for this form. Since it is going to the same department for processing, I would assume you do the same as instructed on form I-129F.

Does anyone else have any insight into these matters?

Drop the 'Urbanizacion El Pacifico', is like putting your subdivision name in your US address. As well, San Martin de Porras is a district and has a 'zip' code: Lima-xx, where XX is a number. Same for work address, Urbanizacion out and Los Olivos has a zip code. If those are the only info, it is ok to use multiple lines, obvisously not filling out the dates columns.

Province or state IS Lima. The capitol city (except Callao) is within the Province of Lima.

Ok thanks, are you sure it is ok to use multiple lines on the computer form as long as you dont add multiple lines for dates?

I would think you do not use multiple lines for one employer. My attachment was simply printed in word exactly like this:

C.2. Address (Number and Street)

CALLE ALEJANDRO VELASCO ASTETE LTE 96B

C.15. Name and address of present employer

MUNICIPALIDAD PROVINCIAL DEL CUSCO, PLAZA CURISPATA S/N, CUSCO
