Greetings,

I'm working on completing the DS-156 forms and have some questions...

1. DS-156 Question 9 - My fiancee does not use other surnames but often goes by her nickname for her first name, should I enter that on the form in box 9?

2. DS-156 Question 14 - What should I put in this box? Is Clave de Elector for Mexico the same as the requested "National ID"? Or would this be the same as CURP number?

3. DS-156 Question 28 - What should I enter here? Most of the transportation will be paid for by me (the petitioner), but we are a team so should I put my fiancee's name down also?

4. DS-156 Question 30 - My fiancee has a B2 / BCC I believe I should enter that in box 30. Should we also include issuances of I-94 forms?

5. DS-156 Question 33 - My fiancee intends to study english in the U.S. but we haven't decided where yet. Should we note this in box 33? Should we enter something like... "Will study english, School undetermined at this time."

Thank you!

1. Only need to list names that she has legally used before.

2. CURP

3. She can put "Myself" or "Myself and fiance"

4. They are only asking when it was issued(date), where it was issued(which consulate) and what type of visa was issued, in her case the B2- boarder crossing card.

5. Just put Unknown.

The example form might help with any other questions about filling it out> http://www.visajourney.com/examples/Form-DS-156.pdf

Thank you for all of the great responses Kay half of Jay-Kay!

I actually have been looking over all of the example forms as I'm filling out ours. The examples are helpful but still leave me with questions. Sometimes I over think things a bit, but I would rather err on the side of caution and prevent delays and problems during the interview.

Best Regards,

Ed (half of Lety and Ed)

You are welcome, Ed! I completely understand. There are times when the example forms do not explain your own unique situation. Ask questions whenever you need to. smile.png
