Help with the new I-90 form...

We are filing an I-90 to request a replacement for my husband's lost green card. He entered the country with a non-immigrant K-1 visa and has since made it all the way through to having a 10-year green card.

In Part 3 "Processing Information" question 3 asks, "Location where you applied for an immigrant visa or adjustment of status." The instructions say to "list the location of the USCIS office where you filed your application for adjustment of status." We mailed our application for Adjustment of Status not to an office but to the Chicago Lockbox like every other K-1 person. That just seems like a weird answer. Does anyone know for sure what is the correct answer? The location of our green card interview is the answer for question 4, so we know that is not the answer for 3.

Thanks for your help. With $450 on the line, we want to make sure we get it right the first time.

I believe it's fine to put Chicago lockbox. That question doesn't seem to work well for the lockbox system. People have that question on the AOS forms and reports have come back that people put the lockbox and others their local office, and all were successful.

Hi I'm currently filing the form I-90 and same questions as above. If I write Chicago lockbox do I need to include the address like PO box? Or can I just write down the address which is the PO box?
