Do I have to put USA too? because I visited 3 times

I don't think living in a country = visiting (as tourists). I believe it applies only when one stayed abroad for at least a year (studying, working etc.). I don't know why would they care to read about anyone's tourist trips. But correct me if I'm wrong.

As im a truck driver and im in europe each and every week I just put the countrys and years to and from on it , Ive never had a stamp in my passport since the UK joined Europe

No, I think at this form you write down all the countries you entered with or without stamp. Thats how other people did that no?

The form says "list all the counries you have entered in the last 10 years". Doesnt say countires you lived in.. I also think they dont care about our holiday trips but i believe that's what they ask for. Maybe what they are interested in is the destinations and unusual patterns etc.

Definitely list them all. They're probably looking for countries they deem unsavoury so they can ask about them later. I wouldn't stress about it though - it's easy to leave out a trip by mistake, particularly when travelling to countries that are nearby/visa-less/stamp-less. It's definitely more than about living in different places, though - so no, they don't care if you went on vacation, but they aren't ask what you were doing there on the form, which is why you list the lot...

Should I write for example 10-2012 Spain or only the year? And I visited Spain but with a transit to Italy. Should I write that like Italy (transit) or leave it?

I only wrote the years and didn't have any problems. I would list Italy as well.

Don't worry too much about it. Try to be accurate, but they won't ever check the coutries you visited (specially european ones inside shengen... )
