Thank you all! I've just spoken with USCIS.

So Part 4 - information US citizen, not my

Part 7 - in First box my signature and make a note that it's mother signature above the box, Print name box - son's name

Spouse can also sign.

Thanks for getting the suggestions checked & confirmed with USCIS and letting us know. This helps us guide others with certainity.

USA

01/08/13 - Approved and GC is order for production on 1/8/14

09/12/13 - Case transferred to CSC. NOA2 received on 09/18/13

08/30/13 - Biometrics Done - No walk ins allowed at this LSC (received on 8/16/13).

08/05/13 - NOA1 (received on 08/10/13)

08/01/13 - Mailed I-751 (received on 8/2/13 - check cashed on 8/5/13)

12/28/11 - Received SSN (applied on 12/20/11, as we didn't get based on DS-230 options)
11/28/11 - Received Green Card (Expires on 10/30/13) - Welcome Letter on 11/17/11
10/30/11 - POE - Houston, TX

Chennai Consulate (40 days)
10/28/11 - Received Visa papers and Passport at VFS
10/25/11 - Interview Cleared Successfully (Spouse was not allowed in)

NVC: (90 days from NOA2 to Consulate)
08/31/11 - Case Completed (Interview 10/25/11) - Received at Chennai on 09/19/11
07/22/11 - NVC Case Number

USCIS: (92 days)

6/21/11 - NOA2 (NOA1 on 3/25/11) - took a month to get to NVC
3/21/11 - I-130 sent to USCIS Lockbox, Chicago.

He is your son and hence you sign for him. It is to hold the parent responsible for the details provided. I suggest that you sign it.

You sign in the "signature" column and lets say your name is "Mary Link" and sons name is "Jim Link", hence "Print Name" should be "Mary Link for Jim Link". Enter "NA" in the Signature of Spouse.

I saw some links where children (little older and are able to write) signed for them selves and then one of the parent singed under "Signature of Spouse" by overwriting "Spouse" with "Mother" or "Father" (as the case may be). I don't recommend this to you, as you son is just 4 years old.

My wife, who is her son's legal parent, signed as folows: Her Signature/Mother

Phil (Lockport, near Chicago) and Alla (Lobnya, near Moscow)

As of Dec 7, 2009, now Zero miles apart (literally)!

One more question. Can somebody tell me what documents put:

I made copies of tax returns, my and son's GC, medical insurance, bills for some Park and Recreation classes

What else i must have for son? I don't think that they need my evidence of real marriage because I have already had my second GC.

Interesting, what evidence do one attach with child'a I-751 when filed separately - does one attach all the evidence sent with parent's I-751 or .... I am not sure and hopefully someone who did before will guide you.
