Hi good afternoon.. we are about to file AOS next week so we preparing all the documents needed to send it to our attorney since my I-94 will be expired this coming 23 and I'm aware that we can still file AOS even after the I-94 expired but our attorney suggested to do that before it expired to avoid some gap on my status. We just notice last night that my St. Luke's Vaccine sheet marked Vaccine History Incomplete and applicant maybe illegible for blanket waiver(s) because vaccination(s) not medically appropriate (as indicated above)several vaccine are marked insufficient time(Tdap,Varicella). My questions are can I submit this copy of St luke's Vaccination Sheet for AOS?? even it says incomplete? or do I need to see civil sergeon? Is there any member here submit the Vaccination Sheet marked incomplete and not having RFE?

Some people submit their St. Lukes vaccination sheet and are fine. Others get RFE's. It's safer to have a civil surgeon transcribe your vaccination sheet onto an I-693. That's what Chinook did. It took about 15 minutes at the civil surgeon's office and cost about $35-40.

Edit: Chinook's vaccination sheet from St. Lukes was just like yours...some vaccinations were marked as not medically appropriate and some were marked as not enough time interval. Technically, your vaccinations are complete for AOS purposes even though the form is marked as incomplete.

I know it may me an extra expense, but I would have a civil surgeon transcribe it. If there is anything missing, you can get it corrected or taken care of before you file. My wife had to get a few extra vaccinations at our local health department (which were free)because her St. Luke's form wasn't exactly complete. In my opinion, it is worth the little bit of extra time, than to worry with an RFE.

We had something like that on my aos, on my sheet so we just went to a civil surgeon and I was required to get another shot, so got it and then got the I-693 in a sealed envelope for the uscis and a copy for my personal records.

04/19/2021 → I-130 NOA1 Mail 

05/03/2021 → Sent through Fedex I-485 (with I-840 and sealed I-693 and I-130 NOA1 copy) I-765 I-131

05/06/2021 → Delivered to USCIS with signature confirmation.

05/25/2021 → Fingerprint Fee Was Received
05/26/2021 → Got text messages for I-485, I-765 and I-131 with receipt numbers.

05/27/2021 → Check cashed.

06/02/2021 → 3 NOAs for I-485, I-765 and I-131 arrived in the mail.

Everything will be fine and hope you found a Doctor that is not charging a ridiculous amount. My insurance covered it. Be sure to use a doctor that is on USCIS that is designated. I talked to a few and seem to be unaware of them being on list or what to do. Be sure to print and bring instruction sheet for doctor and you both go over requirements. You will just receive what you didnt get at ST. Lukes. My wife missed one because I was there during process and fear of being pregnant. Be sure doctor seals and with tamper proof tape and signs the seal.

Check to see if Influenza was checked. For some reason, St Luke's got cheap and didn't give Mike his flu shot although it was in season. He took his medical exam in Oct. His DS-3025 says incomplete + the blanket waiver as well but we went ahead and got his shot for $30 at Vons(could get this cheaper at the community health clinics for $10) and Civil Surgeon transcribed it for $50.

The civil surgeon we went to looked over the ST Lukes sheet and advised us what vaccines she should get. We then brought the record from our clinic of getting the vaccines and the St Lukes sheet and he filled out and sealed the I-693, and gave us a copy just in case they loose it. Guy must have been pretty familiar with USCIS :lol: I know some get through without the civil surgeons report, but we didn't want any delay in getting her green card. USCIS is slow enough as it is.
