Hi guys,

I know, another question about the medical exam for AOS but:

I did the medical exam in Paris to get my visa in July, everything was ok.

the thing is the doc didn't give me any paper, except does that were in the brown envelopp they kept at the POE.

So now what??

i have no copies to submit with the I-485 package, so what should i do?? do i really need to submit a form as they should have the one from the brown enveloppe no??

have another exam here in the US?? call the doc in paris to find out if he can send me a copy of my exam??

i'm getting confused and worried this will take forever :(

Thanks for help guys!!

Same thing happened to me. I didn't get a copy of the vaccination sheet in Germay and the doctor didn't keep one since I was a "one-time-patient". He sent everything off to the consulate with no copies left at his office.

Well, I did go in the US to have another vaccination supplemental filled out and I also had only my German shot records. The doc did it anyway and wanted 50 bucks for it though. He asked me for help every now and then but luckily all those medical words and abbreviations are about the same (at least US-GERMANY) so he didn't have too much trouble with it.

I would still suggest you call the doc in Paris first and see what you can do. If he doesn't have any copie left call the civil surgeons in your area and explain your situation..some might suggest you go to a health department and have it transferred to a internetional shot record (that's what they told me but I didn't wanna go there so I went to the doc right away).

Thanks, that's what i thought...

i do have vaccination records but it's my french records, maybe it's useless...

i'll call the doc in paris then and try to get a copy from him, hopefully he'll have one... or i'll have to go through this again :(

If you are a K-1 and had your medical within a year from filing for AOS, you only need vaccination supplement (I693A) not the entire exam. You should have your french vaccination translated in English, and then to to USCIS approved doctor to transfer it on I693 supplement.

Good luck

That's what i'm gonna do, call first the medic in paris, and then maybe have another exam here, including vaccination shots i guess.

do you know where to find the civil sureons in my area (NJ) ???

thank you ;)

You can find a list of all civil surgeons in NJ here

http://www.uscis.gov/portal/site/uscis/men...00048f3d6a1RCRD

My suggestion..call them all and tell them that you ONLY need the Vaccination Supplemental I639A, don't do the entire medical again, that will cost you a fortune. Even if you have to have your shot record translated into english it will probably still be cheaper than the medical itself. Also, look for doctors with foreign names (I know this is America, everybody is a foreigner) but I have heard that especially doctors from India are more likely to just take your foreign shot record and fill out the vs, although that is not 100percent for sure, either.

Actually, you should be able to use the records from your personal physician in France even if you don't have a copy of the vaccination supplement from the panel physician. I took my personal vaccination records along with the vaccination worksheet to the civil surgeon to have them transferred and he ended up using my original vaccination records more than the vaccination worksheet since he had trouble reading the panel physicians handwriting and couldn't make our her dates. So if you have original proof, that is what they are looking for. The vaccination worksheet overcomes any potential language barriers so you may need to translate your vaccination information from the french for him, but it may be enough.
