Hi thanks for reading and I hope somebody can help me.

Question #2 on the DS-230 - where it says "Other Names Used or Aliases (If married woman, give maiden name)" does that mean if you have been married before I have to put my maiden name and my previous married name? First I just put my maiden name but then I re-read it and it could mean to put my maiden name as well as my previously married name.

Please help as I am just about to submit my paperwork and I don't want any delays.

Hi there, it's a little confusing isn't it. You leave question 2 blank as you are not married, otherwise you would not be eligible to be applying for a K 1 visa. The DS 230 is not just used for K 1 applications, so sometimes the questions may not apply.

I have been married before, however I am divorced so I am therefore not a married woman. So I left question 2 blank. I go by my maiden name, which is what I used at question 1. I then selected Divorced and number of times married 1, even the wording of that question can throw you off as it asks how many times you have been married including your present marriage :wacko: So of course, you would also leave questions 14 to 18 blank....as again these do not apply.

Hope this helps. This form was such a pain, I had to fill in one for myself, which took forever to remember all the address's over the past 21 years and school dates, but I also had to do one each for my 3 children.... uugh.

Tried to edit this reply after I realised your CR1 not K1. Sorry about that,

I would assume you would use your current married name in question 1 and your maiden name in question 2. You could put both and have in brackets (previous married name) next to the old married name. Are you required to submit a divorce certificate from your previous marriage, as this would substantiate the other previous married name.

Hopefully a CR1 applicant will help you out soon.

That is correct, you must write your maiden name and, if married before, must write down that name as well.

Yes. For example for Nicole Marie JONES who was born Smith but first married a Johnson and then a Black, she would enter, "Nicole Marie, SMITH, JOHNSON, BLACK, JONES". From that, the will get the the woman used four family names and the order in which she used them.

Thank you that helps me. As I said I read it just as my maiden name and I put that down and was quite happy with that until days later I re-read it and then started to panic lol.
