Change of address question

I am moving this saturday and I know I have to file an AR-11 form. However I have a question regarding the form: Where it asks me to state my last date of entry into the us, is that the date I listed when I applied for my visa, or my most current reentry?

Also, does my husband have to file an I-865 form as well?
