Hi everyone,

We are having some problems right now figuring out this form I-693. My spouse has had her medical examination before entering the country, and it will be less then a year before filing for the 1-485. The medical report along with the DS 3025 was taken at customs when she entered the US. Does this mean that uscis already has this information and we just have to send in the top part of the I-693?

She was not given her own copy of the DS-3025? Yes, the USCIS has all the medical documents from her K-1 medical(except in those cases where they say they lost them). If you are absolutely positive she had all the required vaccinations and they were marked off on her DS-3025 at the time of her K-1 medical, then you can skip the civil surgeon and the I-693 and worry about all that if you so happen to get an RFE for it.

Do not send in an incomplete I-693 that you filled in yourself. The I-693 needs to be filled out by a civil surgeon. She will not need a new medical, but she will need page 1 and the vaccination page filled out by a civil surgeon. If you have any vaccination records, bring those with you. Did she enter as a K-1? The doctor at her medical didn't give her a copy of it? If not, try to contact him/her, and see if they can email you a copy of it (if they kept it).

Do you have any ideas of which result was on her DS-3025? All her vaccines were up-to-date or were there some missing?

- Apr 13th, 2012 - Visa received - Aug 24th, 2012 - Green Card received

- Aug 8th, 2014 - Green Card received

- Dec 29th, 2021 - Filed online. Got notice that biometrics will be reused.
