Hi all

Have a question. Am about to submit my AOS application and mostly there with the forms. One problem - the medical report. Had a medical last July (05) and came to the US in January 06 on a K1. So i dont need a new medical as its within a year. But i do need to submit a vaccination supplement. I do not have any official documents regarding my medical or vaccinations. I gave them to the consulate in London as instructed and then to the officials at the airport. Presumed they were forwarded on. I have a record of my vaccinations since birth from my doctor. Is that the evidence they need?

Have asked the uscis info line but they said they is no way they can check if there is a supplement in the medical thats been forwarded. I have 2 options - submit and hope they dont ask for a vacc sheet, or go to a doctor and get a supplement done. But i dont want to pay for a full medical?

Has anyone else had this problem, or have any assistance on this matter?

There always seems to be one thing that holds me up - am i alone on this?!?

Yeah, you get the supplement from a civil surgeon here in the US.

Hi Folks, Thanks for the quick responses

Would it be ok if i just went along with the vacc record form my doctor at home, or do i need an official document from the medical i had. Have located a health center around the corner. Im psyched.

I just sent in my vaccination worksheet (DS3025), I didn't submit the I693 (vaccination report from the civil surgeon.) No problems yet - had interview 10 days ago! Good Luck

Everyone's case is different, but here's the USCIS guidelines. It gives you all the info you need and basically states that:

Ks: If you were admitted to the United States as a fiance(e) (K-1), child of a fiance(e) (K-2), Spouse of a U.S. citizen (K-3), or child of K-3 (K-4), and received a medical examination prior to admission, then you do not require another medical examination as long as your application for adjustment of status (Form I-485) is filed within one year of your overseas medical examination. You will, however, be required to submit a vaccination supplement with our adjustment of status application. The vaccination supplement must be completed by a designated civil surgeon.

If you don't have a copy of your Vaccination Worksheet from the 1st medical, phone them and ask them to fax and post you a copy. This will help you avoid having to obtain a 2nd set of the same vaccinations! Also ring around the various civil surgeons to get a good price. Also check that they will fill out the Vaccination Supplemental Form, and not look for you to undergo a full medical, which you do not need.

Thanks for all the help guys. My local health centers tried to convince me that i hadf to go through the whole medical process again, but on the uscis website i found this doctor and rang him. He'd do it for $50 and this saturday. Was starting to get stressed but he said he'd been doing this for 25 years. So im happy.

Is it me, or does this sound like the doctors simply sign the document and make a quick piece of your money?

Well, its done now and i can sleep easy tonight!

They SO just take your money..My husband finally found his vaccination record after he swore that they never gave it back to him. I randomly picked a "approved civil surgeon" from the list and she is going to do it for us for only $15! I was so happy. I get a bit sick thinking about all the money they want for this process. All worth it...just a strain on the pocket for awhile.

Can someone tell me where the list of approved civil surgeons can be found? I am in Cape Cod Mass. and am hoping I have a few choices at least.
