Q 35: ”List all previous visits to or residences in the US. Give type of visa status if known."

How detailed? I've entered 12 times on the VWP, probably another 10 on J-1 and F-1 visas.

1. Should I list each single entry when I was a student? Or should my entire student career be listed as one "visit"? (I entered and returned probably 10 times on the same student visa during the same course of study)

2. "Residences" - Instead of listing each student visa entry, should I just list the different places I lived while I was a student?

3. It says to list "locations" - Do they want street address or just city?

4. How detailed do I have to be on the dates? I was in the US on VWP when I was 2 and 5 years old. I really don't remember the dates, but can narrow it down to an approximate month and definite year.

5. Oh, and I've done this a few times - Enter at LAX on VWP, stamp 90 days. Go to Mexico/Canada for a day, come back on the same VWP trip. Does this count as a new visit? My passport wasn't stamped with a new 90 days, just same return date as the initial stamp.

6. One more thing. It asks for A#. I do have an A number from when I was on OPT. Do I list that along with subsequent entries on the VWP after I received the A#? Or do I just list it while I was on OPT?

List either your only or (if more than one) most recent A number.

On the DS230, I would list only the most recent two entries and then on the third line, either refer to a separate sheet and provide as much information as you can. If ultimately you cannot possibly report all entries, simply indicate "many more entries since 19XX (whatever year you think you first entered the USA) with no overstays." and leave it at that. If there are really too many, just put THAT on line 3 of the entries section.
