I got my credit hours evaluated by the same company. I sent my original documents with the form in which I selected course by course and general evaluation and sent the fee. I requested my report to be sent directly to school I was applying to and one copy for myself. I received all my original documents by mail. Hope that helps :) lemme know u if you have any questions.

Apply first in the school you want to get in, then they will give you the list of requirements for admission. Sometimes, some school doesn't require transcript of records to be evaluated esp if it is written in english. Some school doesn't require toefl if you are employed or your employer could give you a letter that you are fluent with english.
