I would like to know if I can file my AOS without the medical form 693 and just send in a copy of my DS 2035 which was proof of vaccinations given at the US embassy when I arived. I entered on a K-1 Visa and its only been three months since I arrived, cause I read that if you entered on a K-1 and file before one year that you dont need another checkup? Please help.

If your DS-3025 is marked as complete, then you do not need the I-693 form. If it's marked as incomplete, you will need to complete what's missing and have a civil surgeon fill out the form.

Whether it is marked complete or incomplete is not the determining factor for seeing a CS to get an I-693. If you had all the required vaccinations for AOS at the time of your medical, then you do not need to see a CS, even if your DS-3025 is marked incomplete - may be eligible for blanket waiver.

See this post for detailed information about whether or not you can skip the CS: http://www.visajourney.com/forums/topic/335754-ds-3025-complete-or-not/page__p__4971318#entry4971318

Even if you are missing vaccinations, you do not need to get a new medial exam done. You will need to call around until you find a CS that won't insist on giving you a new medical. If you find yourself in need of a civil surgeon, you can try searching through this thread to see if anyone in your area is listed: http://www.visajourney.com/forums/topic/231026-civil-surgeons-who-dont-rip-you-off/
