I-130 Extra page(s) example?

I would like it if someone could xxx out their personal info but post a copy of the "extra page" they made since most of the spaces on the I-130 form do not have enough room for addresses and other data. I would like to follow the example of someone who passed the process rather than risk having the packet delayed for something as stupid as formatting. I searched the I-130 guide and saw nothing along these lines.

This is how I formatted the supplemental sheet on my paperwork:

Top of page and centered: "Form I-130, Petition for Alien Relative; Supplemental Sheet"

I then put the section of the form and then what the question was, directly below, I hit tab once and then entered the information. Example:

Section C, block 2, current address of (your relative):

(Enter address)

As a general rule, when attaching additional pages to a form that is signed and dated, I recommend signing and dating each additional page too.
