My fiance is sending off (or trying to) for his long form birth certificate. He went to his physician today to get guarantor information. When he asked him for his registration number, the doctor wouldn't give it to him! He said that info can not be given out. So now what do we do? This man has been his physicain for YEARS. He is also the only one that my fiance can use for the purpose of guarantor. Has anyone else had this happen? Also, I asked him if his doctor had some form of ID number on his prescription bottle. He did say there was a number that begins with APX-could this be the number??? :help::bonk:

When I applied for mine all I did was put the doctors name and phone number etc. They had no mention at all of needing any sort of number or signature etc.

I applied for mine online.

This is not a consul issue or embassy issue, should be posted in the Canada forums.

Nothing to do with a doctor. Was he born in Canada?

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.

I remember that you can use a teacher as a guarantor so I just listed my cousin who is a teacher (and didn't even tell her) and it was fine.

I don't think you need his number, just his name and phone number.

Guarantor is for a passport... :blink:

You simply apply online for the birth certificate. All you need is your name, DOB, Parents names and DOB as far as I remember...

You need one in Ontario, but it's really not that strict. You just need a name and phone number.

I got my sons Long form birth certifate last year via online and didnt have to put that info... HOWEVER I just looked up the site and found this not to mention it looks different than what I remember..maybe they did so changes to site and what is needed.

I used my cousin who was a teacher. I did have to provide her registration number with the Ontario Teachers Association though. I don't think they ever did contact her but they could have verified her registration number. Would it be helpful if you took the forms to the doctor so he can see it's legitimate and then send it off? Just a thought.

I think they recently relaxed the criteria for guarantors in Ontario- they just have to be a Canadian citizen and have known you for at least two years. I used my uncle as my guarantor in my passport application. They dont't have to be a professional.

FYI:

The APX number is the manufacturer’s/company’s code that made the drug.
