Hi Everyone,

So i had an appointment at the US consulate this morning for a US Claim of citizenship through a US first time passport application. My entire application was accepted except i'm missing either a letter from a physician certifying that he/she provided care to my mother during pre-natal period and at time of birth or hospital records because i was born in an Atlantic province. The problem is i was born 25 years ago, how am i going to go about getting these documents? I'm really worried i won't be able to find them, has anyone experienced this? Is there any way whatsoever around this? please help me.

You are a US Citizen applying for a US passport for the first time in Canada? Or are you a naturalized US Citizen? Are you applying because one of your parent's is a US citizen?

What documents did you present to the consulate?

If you are a US Citizen:

If you were born in the US, you should have a US birth certificate. New requirements for a passport with US birth certificate require you have a certificate that has your parents FULL names. You can get a new certificate by looking up the state you were born in.

If you were born abroad and have US citizenship go here: Replace or Amend a Consular Report of Birth Abroad (CRBA)

IF you are trying to claim US Citizenship because one of your parents is a US Citizen:

You need proof of parent’s U.S. citizenship.

(1) original U.S. birth certificate (long detailed version ) and if available a U.S. passport,

(2) original Certificate of Citizenship or Naturalization, or

(3) original Consular Report of Birth Abroad. Abstracts, short forms or mutilated certificates cannot be accepted.

I'm claiming US citizenship based on my US citizen parent. He meets all the requirements to pass on citizenship to me. My application was approved pending i attain a "Letter from physician certifying that he/she provided care to my mother during pre-natal period and at time of birth or hospital records". The problem is i was born 25 years ago, what are the chances of the hospital still having any records of my mother during her pre-natal period??

My father meets the requirements to pass US citizenship to me, his 25 year old son. My application was approved, however i'm required to attain one more piece of documentation. The US consulate asked that i provide either "A letter from physician certifying that he/she provided care to my mother during pre-natal period and at time of birth" or "Hospital Records". Now, my concern is how do i go about attaining these pieces of documentation that are older then 25 years. Also, why am i required to provide this information? I'm so drained of emotion :(

Your father is the US citizen?

His name is not on your birth certificate?

I'm not sure why they would need your mother's medical records for your care if your father was the Citizen.

The only thing you can do, if they want this evidence, is to call the hospital you were born in and ask if you can get a copy of your birth records.

As I stated in the previous post you put on here...

If this is the documentation they want, call the hospital you were born in. The records may be in storage and it will probably take a while to find them and you will probably pay a search fee.

The reason they are probably asking for this information is 1. you're parents were not married when you were born or 2. for some reason you do not have your fathers Birth Certificate proving he is the US citizen.

My parents were married, both their names were on my long form birth certificate. Can i just do a DNA test to prove my mother is my mother? will that satisfy their requirements? I provided all the correct documentation, this is the documentation i was missing. I'm basically going through a CRBA process, except i'm applying for a US passport because i'm over 18 years of age now.

Ok..

Your father is the US Citizen. His name is on your birth certificate.

Your mother is Canadian?

And you were born in Canada?

When you were born your parents did not file at the US consulate about your birth abroad to a US Citizen?

I don't mean to be pushy, just trying to help you the best I can. The more info, the better I can help.

No they didn't file my CRBA, now i'm over 18 and cannot attain a CRBA, but i can still claim my citizenship. I applied for a US passport this morning, everything went ok, but they just need these documents. Do you think the hospital will still have these records after 25 years? What options am i left with if they don't? The purpose of these records is to ensure my mother gave birth to me.. so can't i just go do a DNA test to prove that?

I think I have this figured out. The reason they are asking for the hospital records is because you need to prove your father is your father and your mother is your mother. Silly, I know.

They way they want you to prove this is by your birth records from the hospital. The best you can do is do a web search for the hospital you were born in and call them. Like I said before, you may be in for a long wait. And it could be expensive.

I think you have a good shot at it. Most hospitals keep records for 10 years then they are put in storage. Some put them on microfiche. I'm sure they are in storage somewhere and with a little bit of work can be found.

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.

I put in months of preparation into this application, i never would have thought i would be required to submit these documents.. The worst thing about it is because i was born in an Atlantic province in Canada i'm required to submit this information, otherwise everything would be alright. I'm going to call the hospital monday and hope for the best. Thanks for your help.

I don't understand why they aren't satisfied with your Cdn birth certificate? If it lists both your parents what is the problem?

In any case, I wish you good luck - contact the hospital and failing that perhaps the province has some sort of documentation that was submitted with your birth.

Do they want this information as documentary proof of my mother's pregnancy? Can't i just go do a DNA test and that will lay this to rest? I think a DNA test would be more valuable evidence then prenatal or ultrasound records that she was pregnant with me. I'm so confused...
