I need to obtain a child abuse history clearance and do a criminal record check with fingerprinting. Is this something I can already do with just the ead or would I need the GC? I already have my SSN too. Would I use my maiden name (on my passport) or my married name (on ead and other Aos forms)? Thanks in advance for your responses!

My Journey:

We met through a study-abroad program in Shanghai, China in August of 2009

We got engaged March of 2010

I received my K1 VISA in 6 months (June-December 2010)

We were married 04/02/2011
I received my conditional 2-year greencard (AOS) in 2.5 months with no interview (April-June 2011)

Our son was born 02/03/2013

I received my masters degree in Speech-Language Pathology 04/17/2013

I received my 10-year greencard (ROC) in 3 months with no interview (March-June 2013)

My husband returned from deployment 06/20/2013

My naturalization journey took 4 months (April-August 2014)

I became a US citizen on 08/01/2014

Received passport in 3 weeks (regular processing)

Thank you, VJ! smile.png

If this is state specific (not needing federal check or clearance), I THINK you need the state ID number as well, issued either as state id card or drivers licence .

Most jurisdictions that record this stuff use SSN and state ID #.

You should have some 'intake form' already - what are the instructions for ID intake?

R u serious ??

You need a background check for getting admission in the Grad school? WOW!!!!

From your original post I thought you needed a clearance for job or working with child care facility.

Well if your school is making to do it, they can prob also tell you what agency to get the clearance from and what are the forms you need to fill.

In case of using which name, your married name would be the name on the form and then most form have room for other alias that one has used and you can put your maiden name in there.

Thank you! Yeah the email was for all admitted students so thank goodness its not just for me. :) I was told that the fbi criminal record and child abuse history clearance could be applied for online and that I would just need the SSN. Phew!! They are also making me get a BLS for healthcare providers certification from the american heart assoc. Not sure if these are school requirements it might be requirements of the American Speech and Hearing Association or ASHA for us to get licensure. Either way all these paperwork is making me nervous, I just feel like it never ends!
