No Immunization Records

I'm worried that what my doctor provided me with isn't going to be sufficient. There's no way to get my previous records, so when I presented him with the information I would need, he ordered a blood test.

I was given back a sheet of paper that shows immunity for Measles, Mumps, Rubella and Hep B. And then a one line printout saying he gave me a tetanus shot in January.

I'm probably worrying over nothing (It's been known to happen) but even after I pressed my doctor he said that was all they'd need (I'm a 26-year old female). I was hoping someone might have an idea if I'm missing any documentation here? I haven't booked my medical yet because I need to get my work schedule for June/July first, but I want to make sure everything is in order when I do.

Based on the Panel Physician's immunization page - it says for each immunization if you can provide 'An antibody titers blood test showing proof of immunity of ...' you're fine.

You didn't mention anything about Chickn pox or Tetanus Diphtheria Pertussis or the Flu shot - what about those?

Based on the Panel Physician's immunization page - it says for each immunization if you can provide 'An antibody titers blood test showing proof of immunity of ...' you're fine.

You didn't mention anything about Chickn pox or Tetanus Diphtheria Pertussis or the Flu shot - what about those?

Is there any reference to the titers blood test in the DOS site? Asking because my fiancee is in same situation, no vaccination card (still several weeks to interview, but don't want to wait to the last minute)

I had chicken pox when I was in grade 4, and I remember reading that verbally confirming that was enough? It's outside of flu season so I think that's covered and my Tetanus shot one just says "Tetanus shot" so is that different one I need?

It depends if they used TDAP or ADACEL, they are both the same really just different companys, often doctors just day like " tetanus " so just ask your DR if it was either of thos ones.

Oh, I'm sure I've gotten everything! I just have no way of proving it! That's what I'm worried about, not having sufficient documentation for the medical.

Before I went for my medical(about 3 1/2 years ago) I didn't have an immunization record..but I was told if I called public health(through the board of education) I could get copies...it took a little bit of tracking down the right office..but they found my school records and faxed it to me..just a thought being you have time before your medical.

In Ontario, the public health schedule only uses the tetanus/diptheria/pertussis shot at the 14-16year checkup, after that you only get tetanus/diptheria, and the vaccine is called Td Adsorbed, it's NOT the same as Adacel, as it doesn't have the pertussis in it. Yes, you would have had to have the childhood immunizations in order to go to school, but public health may not have all your shots on file, though they should. I constantly get calls at the doctor's office from parents asking if their kids are up to date because public health says they're not, and 9 times out of 10, they've had the shot but the parents never notified public health, so if your parents were anything like the majority of parents, then they might not have kept public health updated with all your shots. At the very least, at your medical, if they feel you need Adacel, they'll give it to you there, for a fee of course!
